Complete Medication Review, Delivery & Oversight
A Clinical Pharmacist begins by reviewing medications you are prescribed, how you take them, and how you order and receive them. We will then safely consolidate your medications at a Nebraska Enhanced Service Provider Network Pharmacy. Your new medication plan will then be delivered to your door automatically in pre-packaged Adherence Packaging.
Personalized Health Screenings & Wellness Planning
We'll then give you a screening for vaccinations to keep you strong and healthy. After the screening, a personalized medication plan will be presented to both you and your Primary Care Provider (MD, NP, PA). We work closely with you and your family to develop a care plan specific to your wellness needs.
Ongoing 1-on-1 Health & Wellness Planning
After a medication plan is in place and your screenings are complete, a Clinical Pharmacist will accompany you on your next Primary Care Provider visit. We collaborate with your Primary Care Provider to fully integrate the next steps in your wellness. This personal care can continue as needed to improve medication adherence.
