A Clinical Pharmacist will meet you in your home and review all your medications, both prescription and over-the-counter.

All medications will be documented and carefully audited under the following criteria:

• How do the medications interact with each other?
• Are any of the medications are duplicated?
• Are all the medications necessary to properly treat the symptoms?
• What are the side effects of each drug?

If managed correctly, many medication plans can be consolidated, simplified, and made safer by lowering the amount of drugs needed.

After your medication review, we will safely consolidate your medications, specific to your needs, at a Nebraska Enhanced Service Provider Network Pharmacy.

Your new medication plan will:

• Arrive at your door automatically each month.
• Be delivered in easy to read, pre-packaged Adherence Packaging.
• Include a simple, easy to follow medication schedule.

We Provide a Personalized Health Screening and Design a Wellness Plan Specific to Your Needs

A Clinical Pharmacist will conduct a personalized health screening, including:

• Seasonal flu shot
• Pneumonia vaccination
• Shingles prevention

You will also be screened for concomitant therapy, such as:

• Hepatitis B shots for diabetics
• ACE inhibitor therapy for diabetics

After the screening, you and your Primary Care Provider will be given a written wellness plan.

We work closely with you, your family, and your Primary Care Provider to develop a care plan specific to your wellness needs, including:

• Medication reconciliation (adding or subtracting medications)
• Medication synchronization (get all fills at one time)
• Adherence packaging
